The fields that appear on this tab are defined below.
| Surface | Surface indicates the type of surface that appears with a distress type. On this tab Surface will always be Portland Cement Concrete. |
| Name | Name stores a number for a distress type. |
| Description | Description displays a description of the distress type. |
